HIPAA Notice of Privacy Practices
AccuMix Pharmacy, LLC · 655 W Grand Ave, Suite 240, Elmhurst, IL 60126 · 866-817-5303 · privacy@accumixrx.com
Effective date: October 14, 2026.
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please read it carefully.
This notice covers the health information AccuMix Pharmacy, LLC (“AccuMix”, “we”, “us”) creates or receives when we fill your prescriptions: your name and contact details, your prescriptions, the medications we dispense to you, your allergies and the other health information your prescriber gives us, and your payment records. We call this your health information. We follow the HIPAA Privacy and Security Rules and Illinois law in handling it, and our website’s Privacy Policy covers information that is not health information.
Your rights
You have the right to:
Get a copy of your records. You can ask to see or get a copy of your prescription and payment records, on paper or electronically. Ask in writing or by phone; we will provide it within 30 days and may charge a reasonable, cost-based fee for copies.
Ask us to correct your records. If you think something in your records is wrong or incomplete, ask us in writing to correct it. We may say no, but we will tell you why in writing within 60 days, and you may add a statement of disagreement to your record.
Ask for confidential communications. You can ask us to contact you in a specific way (for example, only by mobile phone, or only by mail to a particular address) and we will honor reasonable requests.
Ask us to limit what we use or share. You can ask us not to use or share certain health information for treatment, payment or our operations. We are not required to agree if it would affect your care, but if you pay for a medication in full yourself and ask us not to share that information with a health plan, we will honor that request unless a law requires us to share it.
Get a list of those with whom we have shared your information. You can ask for an accounting of the times we have shared your health information in the six years before your request, who we shared it with and why. It will not include disclosures for treatment, payment or our operations, or those you asked for. One accounting a year is free; we may charge a reasonable fee for more.
Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to receive it electronically.
Choose someone to act for you. If you have given someone medical power of attorney, or someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will confirm the person’s authority before we act.
File a complaint if you feel your rights are violated. You can complain to us using the contact details at the end of this notice, and to the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/privacy/hipaa/complaints, by calling 1-800-368-1019, or by writing to 200 Independence Avenue SW, Washington, DC 20201. We will not retaliate against you for filing a complaint.
Your choices
For the following, you can tell us what you prefer, and we will follow your instructions:
- sharing information with your family, close friends or others involved in your care or in paying for your care, and
- sharing information in a disaster-relief situation.
If you are not able to tell us your preference, for example because you are unconscious, we may share your information when we believe it is in your best interest.
We never share your health information for marketing purposes or sell it, and we do not use it for fundraising. We do not keep psychotherapy notes.
How we use and share your health information
To fill your prescription and care for you. Our pharmacists and staff use your health information to check and fill your prescription, to counsel you about your medication, and to keep your prescription records. We share it with your prescriber and the clinic or telehealth program that sent your prescription, for example to verify or clarify an order, to ask for a refill or to report a problem, and with another pharmacy if you ask us to transfer a prescription.
To bill for our services. We use your information to collect payment for your medication from you or, when a clinic or telehealth program pays for its patients’ prescriptions, from that program. We do not bill health insurance.
To run our pharmacy. We use your information for quality checks, staff training, licensing and accreditation reviews, and to answer your questions and complaints.
To contact you about your medication. We may call, text or email you about your prescription, a refill, a shipment or a question from the pharmacist. Tell us if you want us to contact you a different way.
Through companies that work for us. We share information with service providers that help us run the pharmacy, such as our pharmacy software vendor, our shipping carriers and our phone and email providers. Each one signs a business associate agreement that requires it to protect your information and to use it only for our purposes.
As the law requires or permits. We may also share your health information:
- with public health authorities, and with the FDA for medication recalls and reports of side effects;
- with state boards of pharmacy, the DEA and other health oversight agencies for licensing, inspections, audits and investigations;
- in response to a court order, subpoena or other lawful process;
- with law enforcement in the limited situations the law allows;
- to prevent a serious threat to the health or safety of a person or the public;
- for workers’ compensation claims;
- with a coroner, medical examiner or funeral director as needed;
- for specialized government functions such as military or national security activities, and for the care of people in custody.
Any other use or sharing not described in this notice will be made only with your written authorization, which you can revoke at any time in writing, except to the extent we have already acted on it.
Some information has extra protection under state or federal law, such as information about mental health treatment, substance use disorder treatment, HIV status or genetic testing. Where those laws apply, we follow the stricter rule.
Our responsibilities
We are required by law to keep your health information private and secure, to give you this notice of our duties and privacy practices, to follow the notice that is currently in effect, and to tell you promptly if a breach occurs that may have compromised your information. We store prescription records in our pharmacy system, which is protected by access controls, encryption and audit logs, and we keep records for as long as Illinois and federal pharmacy law require.
Changes to this notice
We can change this notice, and the changes will apply to all information we have about you. The current notice is always available on our website at accumixrx.com and on request from the pharmacy, with its effective date at the top.
