For patients
Refer your provider to AccuMix Pharmacy
We compound medications only on a prescription from a licensed provider. If your doctor, nurse practitioner or physician assistant has not worked with us before, there are two simple ways to connect us. Your provider always makes the decision.
Two ways to connect us
Choose whichever is easier for you
We contact your provider
Fill in the short form below. A pharmacist or a member of our team calls or emails your provider’s office, introduces AccuMix and answers their questions.
Bring a one-page sheet to your visit
Print or download our provider sheet. It tells your provider who we are, what we compound and how to send a prescription.
How it works
From your request to your prescription
- You tell us who your provider is.We ask for your name and date of birth so the office can find your chart, and which medication you would like to discuss.
- We contact the office.By phone when we have a number, otherwise by email. We tell them you asked us to get in touch.
- Your provider decides. If a compounded medication is right for you, your provider sends us the prescription.
- We compound and ship.A pharmacist verifies the prescription, we compound it for you, and it ships to you. Our turnaround time is 24 hours.
What to expect
A few things to know first
- A prescription is required. We cannot dispense without one.
- Whether a medication is right for you is your provider’s decision. Your provider may say no or choose a different treatment.
- We do not offer anything in exchange for a referral, to you or to your provider.
- We can serve patients only in states where we hold a pharmacy license. See the states we serve.
- No provider yet? Contact us and we can coordinate with a licensed telehealth provider in your state.
