  • Different strength or titration is required (including micro-doses).
  • Alternative dosage form is needed (e.g., liquid instead of tablet; transdermal cream/gel; troche/lozenge; suppository; sterile injection or ophthalmic).
  • Problem ingredients must be avoided (dyes, lactose, gluten, alcohol, certain oils).
  • Compatible medications need to be combined into a single preparation to simplify use.
  • The commercial product is on shortage or discontinued.
  • Pediatric/geriatric patients need customized dosing or easier administration.
